Mental Health Worker / Case Manager

A local behavioral health organization is seeking a Mental Health Worker / Case Manager to join their team on a per diem basis to cover nighttime hours and serve as a backup to evening and weekend staff.

The primary duty of the Mental Health Worker is to provide crisis stabilization and crisis support management to individuals experiencing immediate and often life-threatening crisis situations in a residential setting. The ideal candidate for this position can demonstrate a welcoming, empathetic, and integrated approach to their role. Candidates must also have advanced training in a mental health profession (i.e., nursing, social work, psychology) and strong clinical skills.

Responsibilities

• Conduct comprehensive assessment of psychiatric history, mental status, and diagnosis.
• Provide crisis stabilization and crisis support management based on risk assessment.
• Develop, implement, evaluate, and revise crisis plans in conjunction with the other members of the crisis team.
• Provide individual crisis stabilization and crisis management, ensuring immediate changes are made in the crisis plans as clients’ needs change.
• Provide symptom education to enable clients to identify their mental illness symptoms.
• Educate and support clients' families, and advocate for clients' rights and preferences.
• Document client progress to maintain a permanent record of client activity according to established methods and procedures.
• Provide support, advocacy, and coordination of social, vocational and activities of daily living needs.
• Attend team meetings and training on a regular basis.
• Perform other duties as requested.

Qualifications

• Bachelor's degree in Social Work, Nursing, Counseling, Psychology or related field.
• Current certification in CPR and First Aid.
• TB clearance.
